The Minnesota Department of Health concluded its investigation Oct. 30 after finding the claims made against Travis to be substantiated.
Travis is no longer employed by Heritage Haven, Inc., an assisted living facility at 3044 Morris Thomas Road, in the Piedmont Heights neighborhood.
In June, a 71-year-old resident reported to the Minnesota Department of Human Services that multiple unauthorized charges appeared on her debit card.
Thirty-five transactions totaling $8,401.97 were made between Jan. 16 and May 25 to IL Makiage, a New York-based cosmetics company. The products were ordered using Travis’ name and shipped to her home address.
Travis was selling the beauty products on her Facebook account and the transactions were reported to the Duluth Police Department.
Travis is scheduled for an omnibus and pretrial hearing Tuesday.
